Are you having issues with your patient portal?

STEP 1:
You will receive an email that will allow you to activate your patient portal.
STEP 2:
Then, you will enter in your birthdate and last name.
allow you to activate your Patient Portal. Click “Verify Information”.
STEP 3:
You will need to create a password of your choice.
Click “Set Password”. Then, you be granted access to your Patient Portal.
STEP 4:
If you are filling out paperwork on the PocketPatient App,
- Complete ALL the sections at the bottom of the app
- Click “My Health” at the bottom
- Fill out ALL sections:
- Demographics
- Past Medical
- ENT
- Social
- Family
- Past Diagnosis
If you are filling out paperwork on the computer,
- After you login, click the three lines in the top left corner
- Click “My Health” at the top
- All the sections on the left need to be filled out:
- Contact Info
- Insurance and Pharmacy
- Medications
- Allergies
- Past Medical History
- ENT History
- Family History
- Problem List
- My Forms
If you are filling out paperwork on a mobile web browser,
- After you login, click “My Health” at the top
- All the sections on the left need to be filled out:
- Contact Info
- Insurance and Pharmacy
- Medications
- Allergies
- Past Medical History
- ENT History
- Family History
- Problem List
- My Forms
