Are you having issues with your patient portal?

Hearing loss patients at higher risk of hospital readmission
STEP 1:

You will receive an email that will allow you to activate your patient portal.

STEP 2:
Then, you will enter in your birthdate and last name.
allow you to activate your Patient Portal. Click “Verify Information”.

STEP 3:
You will need to create a password of your choice.
Click “Set Password”. Then, you be granted access to your Patient Portal.

STEP 4:
If you are filling out paperwork on the PocketPatient App,

  • Complete ALL the sections at the bottom of the app
    • Click “My Health” at the bottom
    • Fill out ALL sections:
      • Demographics
      • Past Medical
      • ENT
      • Social
      • Family
      • Past Diagnosis

If you are filling out paperwork on the computer,

  • After you login, click the three lines in the top left corner
  • Click “My Health” at the top
  • All the sections on the left need to be filled out:
    • Contact Info
    • Insurance and Pharmacy
    • Medications
    • Allergies
    • Past Medical History
    • ENT History
    • Family History
    • Problem List
    • My Forms

If you are filling out paperwork on a mobile web browser,

  • After you login, click “My Health” at the top
  • All the sections on the left need to be filled out:
    • Contact Info
    • Insurance and Pharmacy
    • Medications
    • Allergies
    • Past Medical History
    • ENT History
    • Family History
    • Problem List
    • My Forms
